Born Moss Side, Manchester. Son of F. W. and Helen Edith Maddocks. On attestation, his father Frederick William Maddocks lived at "Old Trafford," 23 Muston Street, Mosman.
His father later moved to Wolseley Road, Mosman.
Wounded 15 November 1915, Gallipoli, bullet wound - chest/shoulder. Died, enteric fever, 1st Australian General Hospital, Heliopolis, Egypt.
